COVID-19 Vaccination Registration Instructions

Healing Hands Ministries is offering the COVID-19 vaccine to qualifying individuals eligible for Phase 1A and Phase 1B. You do not have to be an HHM patient to receive the vaccine, but you must complete the 2-step process to be entered into our database. Please follow the directions below.

  • Click “COVID Vaccine Registration” at the top of the webpage
  • You will be taken to the COVID-19 Vaccination Registration form webpage
  • Please fill out the form with your Name, Phone Number, and Email. On this form, also indicate the criteria in which you qualify for the vaccine at this time.
    *If you are a healthcare worker, you much list your place of employment
  • Once form is complete, click the green SEND button.
  • After clicking SEND, you will be redirected to our Patient Portal login page. Click the button in the bottom right-hand corner of the page that says “Pre-Register”.
  • Please Only Register Once
  • You will enter all required Personal Details, Emergency Contact, and Employer information (optional)
  • Once you successfully complete all the steps required for registration, you should receive a screen that says:
    “Thank you. You have been Successfully Registered”
  • You have now completed registration into the Healing Hands Ministries database to receive the COVID-19 vaccine. You will receive a phone call by Healing Hands to be scheduled for your appointment. Please be patient as we will do our best to give out vaccines as we receive them.
  • You Do Not Need To Register More Than Once
